Breast Implant Placement

In this position, the implant is placed beneath the pectoralis muscle superiorly but lies directly under the breast tissue inferiorly. The dual plane position allows Dr. Chapin to deliver accurate implant placement and provides the best shaping. With this position, the patient benefits from the advantages of submuscular position, such as decreased visibility and reduced risk of capsular contracture. Dual plane position is also ideal for patients with a small amount of breast sag to give a slight lift to the breast, avoiding the need for a breast lift. This is Dr. Chapin’s preferred method, as it is typically the most beneficial to the patient. Although dual plane position offers several advantages, many surgeons don’t offer this specialized technique. Dr. Chapin, who has extensive experience in this procedure, is part of the small minority of plastic surgeons who perform dual plane implant positioning.

This position places the implant behind the pectoralis muscle for a more natural appearance. The muscle helps mask the implant and provides a smoother appearance to the breast. Typically, this position provides more support to the implant, meaning it’s less likely to stretch the breast tissue over time. With this placement, patients achieve more accurate imaging and have a decreased risk of capsular contracture.

This position places the implant in front of the muscle and behind the breast tissue. The advantage of this placement is an increased volume to the area of the breast behind the nipple. However, its disadvantage is that this placement makes the implant more visible and felt. Additionally, there is an increased risk of capsular contracture with this position.

Breast Implant Incision Options

In breast augmentation surgery, Dr. Chapin employs the inframammary incision, which is made right under the breast fold. This site is the most common and often the optimal choice for most breast augmentations because it reduces risk of capsular contracture and breastfeeding complications and allows for a straightforward procedure.

Breast Augmentation Surgery Day Overview

Dr. Chapin performs all breast augmentation procedures in a Bucks County or Montgomery County AAAASF-certified cosmetic surgery facility.

On the day of your breast augmentation, you’ll arrive approximately an hour before your scheduled surgery to meet with Dr. Chapin and your anesthesia provider. Dr. Chapin and his team will review the surgery details and answer any final questions you have. You will be administered either IV sedation or general anesthesia as determined during your consultation.

According to your procedural plan, Dr. Chapin will begin your breast augmentation by creating a small incision to insert the breast implant into the breast pocket with the Keller Funnel. The entire procedure typically takes between one to two hours.

Breast Implant Recovery and Healing

As breast augmentation is an outpatient procedure, you will need someone to drive you to and from the surgical facility and stay with you for at least the first 24 hours. 

Following breast augmentation, the incision sites will be dressed, and a surgical bra will be placed to minimize swelling and provide support. You will need to wear a supportive bra, that does not contain any underwire, for several weeks as you heal.

You may experience pressure or discomfort for one to three days following your procedure, and you may find it difficult to lift your arms.

Typically, patients take a few days off from work following breast augmentation. For at least two weeks, you should avoid strenuous exercises, but Dr. Chapin recommends short, light walks to promote blood circulation and healing. Dr. Chapin will monitor your progress and let you know when to resume all daily activities, including exercise, typically in three to four weeks.

  • Stop Certain Medications or Supplements. For two weeks prior to your surgery, refrain from taking blood-thinning medications, anti-inflammatory drugs, or herbal supplements that may cause additional bleeding.
  • Eat Balanced Meals. Don’t crash diet or change your diet significantly prior to surgery. However, eat healthy and balanced meals prior to your breast augmentation, as proper nutrition supports healing.
  • Prepare for Recovery. Prepare for your recovery by purchasing the supplies you’ll need and filling pain medication prescriptions. Consider cooking and freezing meals for easy preparation. Arrange a clean and comfortable space to rest as you heal. Also, plan for a friend or family member to drive you home following your procedure and stay with you for at least the first 24 hours.

The Keller funnel will assist Dr. Chapin in effectively placing the implant into the breast pocket. The Keller funnel allows for a smaller incision and reduces the risk of infection and capsular contracture.

A Keller funnel is used to perform the “no-touch technique.” Dr. Chapin will push the implant through the funnel instead of directly touching the implant. Therefore, this reduces the risk of infection and other complications.

GalaFLEX is an internal mesh that provides extra strength to the breast tissue in order to reinforce the placement of the breast implant. Typically, patients with a significant loss in their skin elasticity benefit from the placement of GalaFLEX.

At the time of your consultation, Dr. Chapin will examine you and will personalize a plan specific to you. A breast lift is desirable when your nipple position is below the level of your breast fold.

No, you will not have drains after a breast augmentation.

Yes, after a breast augmentation you will have two sutures— one under each breast— that will be removed 7 days following surgery.

Yes, there will be a small scar hidden in the bottom fold of your breast. These scars heal and fade nicely and are very well hidden.

Yes, breast augmentation requires general anesthesia.

Yes, you will need a responsible adult to drive you home from the surgery center and accompany you the first 24 hours postoperatively.

Yes. We will provide you with a surgical bra (or a supportive sports bra of your choice) which you will wear around the clock for the first four weeks following surgery.

Typically, most breast augmentation patients will notice enhancements to their overall figure immediately after surgery. However, you may also experience swelling that temporarily obscures your results. It may take three to four months for residual swelling to subside and for your implants to settle, revealing your full results.

You will experience long-lasting results from your breast augmentation by maintaining your healthy diet and exercise regimen. Breast implants are not considered lifelong devices, and as such, they should be replaced as often as recommended by your plastic surgeon.

Recovery is different for everyone, but typically patients tolerate breast augmentation very well. Your chest may feel tight for the first 72 hours following surgery, but the pain is well managed with Exparel and oral medications. You will be up and moving immediately following surgery. You will be able to engage in light activity such as walking or riding a stationary bike 48 hours following surgery. By week 4, you will resume your full activity.

Yes, you are encouraged to obtain your annual mammograms after breast implants are placed. Technicians are experienced in performing mammograms on women with breast implants.

There is a 50% chance that you will want to have another breast surgery, in the future, if you decide to undergo a breast augmentation. This could be due to complications such as breast implant rupture or capsular contracture, or it could be because of your desire to enhance your breast shape and size as you age.

Complications of breast augmentation include the risk of bleeding, infection, capsular contracture, sensitivity changes, alterations in the ability to breastfeed.

Typically, this is not an issue for our patients, but it is possible. Some patients lose sensation to their nipples while other patients gain sensation. This could be temporary or permanent.

Yes, breast implants can rupture. If you have a saline breast implant that ruptures, it is more of a ‘social emergency’ because the ruptured breast will deflate. If you have a silicone breast implant that ruptures, you won’t be able to tell on a physical exam. An ultrasound or MRI will confirm that a silicone implant has ruptured.

You will be able to move immediately following your breast augmentation surgery. You likely will not hurt anything by performing basic movements. However, it is important that you take it easy and listen to your body.
